Circunferência da panturrilha como preditor de ausência de sarcopenia em idosos institucionalizados. Um estudo transversal

Introdução: A sarcopenia é caracterizada pela redução da massa muscular que acontece durante o processo de envelhecimento, e a circunferência da panturrilha tem sido utilizada para determinar esse desfecho em diversos estudos. Objetivo: Determinar o ponto de corte da circunferência da panturrilha como preditor de ausência de sarcopenia em idosos institucionalizados. Desenho e local: Estudo transversal conduzido em Instituições de Longa Permanência para Idosos de São Caetano do Sul. Métodos: Foram avaliados 139 idosos institucionalizados de São Caetano do Sul. A sarcopenia foi caracterizada pela reduzida massa muscular combinada com reduzida força muscular e utilizou-se a circunferência da panturrilha como preditor de ausência de sarcopenia. A definição do ponto de corte da circunferência da panturrilha foi determinada por meio da curva característica de operação do receptor (Receiver Operating Characteristic, ROC). Foi identificada a área total e seus respectivos intervalos de confianças (IC 95%) sob a curva ROC, além da sensibilidade e especificidade entre a circunferência da panturrilha e a ausência de sarcopenia. Resultados: A área da curva ROC foi de 0,83 (IC 95%: 0,70-0,96) para os homens e 0,84 (IC 95%: 0,74-0,93) para as mulheres. O ponto de corte na circunferência da panturrilha foi de 28,9 cm para os homens e 28,7 cm para as mulheres, apresentando sensibilidade de 86% e especificidade de 71% nos homens e 82% de sensibilidade e 70% de especificidade nas mulheres. Conclusão: O ponto de corte apresentado é um importante preditor de ausência de sarcopenia em idosos institucionalizados, medida de baixo custo e alta aplicabilidade.
